Nurse Navigator

My Childhood Cancer Navigator Program (Full-Time - Coral Gables, FL)

Reports to: Program Director

About SebastianStrong Foundation & My Childhood Cancer Navigator

SebastianStrong Foundation was created in memory of Sebastian Ortiz, a kind, funny, and deeply loved teenager who passed away from rhabdomyosarcoma at age 16. His only treatment options were decades old. Today, we work to change that by funding less toxic, more targeted treatments for childhood cancer and by supporting families navigating the challenges that come with it.

Our My Childhood Cancer Navigator program provides clear, compassionate, and personalized support to families across Florida. A small, dedicated team that includes a nurse navigator, social worker, and patient advocate provides non-clinical guidance across treatment, logistics, emotional well-being, and other areas. Services are delivered remotely and tailored to each family’s needs, whether that means understanding insurance, finding housing support, or simply knowing someone is checking in.

Position Overview

The Nurse Navigator helps families better understand their child’s diagnosis, treatment roadmap, and available care options. While not involved in clinical decision-making, this role focuses on translating complex medical information into clear, accessible guidance. By providing education around terminology, clinical trial basics, side effect management, and provider communication strategies, the Nurse Navigator helps families feel more confident and prepared. This is a non-clinical role grounded in trust, consistency, and medical literacy. Working closely with the Social Worker and Patient Advocate, the Nurse Navigator contributes to a coordinated team approach that supports the whole family.

Although services are delivered remotely by phone, Zoom, and email, this position is best suited to someone based in South Florida who can collaborate in person with the team at our Coral Gables office. We are a small, mission-driven organization that values responsiveness, warmth, and teamwork. Regular check-ins, case discussions, and trainings take place both virtually and in person. Part-time or remote arrangements may be considered for the right candidate.

Key Responsibilities:

  • Translate diagnoses, treatment plans, terminology, and procedures into clear, accessible language for families

  • Provide education on non-clinical care topics such as side effect management, infection precautions, symptom monitoring, medication organization, and daily wellness strategies

  • Help families prepare for clinical visits by identifying key questions and organizing follow-up steps

  • Offer guidance on clinical trial basics, second opinions, and navigating care across institutions

  • Refer families to credible resources (e.g., ClinicalTrials.gov, nonprofit guides) and help them make informed decisions

  • Stay current on innovations in pediatric oncology and maintain an updated internal reference library

  • Maintain regular check-ins with families throughout their cancer journey, including during treatment, survivorship, or bereavement. Follow up on referrals to ensure families are successfully connected to services and adjust support plans as needed

  • Develop and deliver virtual or in-person educational sessions to help families better understand treatment and supportive care topics

  • Serve as the rotating Intake Navigator during assigned weeks, conducting comprehensive assessments and coordinating team support

  • Represent the Navigator Program at community events, outreach activities, and resource fairs

  • Help gather stories, testimonials, and family feedback that reflect the voices of those we serve, supporting broader awareness and fundraising efforts

  • Maintain accurate, confidential documentation of services, outcomes, and family needs in the secure CRM; contribute to the online resource portal and share updates during team meetings

  • Collaborate closely with the patient advocate and social worker to ensure coordinated, compassionate, and responsive care across the team
